For Local Workforce Board and TWC staff. Use this form to request a new tablet for a new provider, or additional tablets for an existing one.
The KinderSystems Tablet Team reviews every request. Once approved, if the provider has not signed the TWC Device Agreement yet, it is sent to the Controlling Person email you enter.
Please don't include personal information about children. We use this information only to process the tablet request.
